[Submissions] Update "Replace PDF" - allow for adding pdfs to empty fields & warn before clearning

Currently, our systems fail to function if one clicks the "clear" button on the attachment widget and then tries to replace the pdf. The tool should be robust against these accidental removals and warm before the clear is executed (if possible)
